Manual High School, established in Whittier in 1894, became a central public facility for the neighborhoods, and a beloved institution for many residents long after they had departed once familiar streets.ĭenver’s continued growth altered the fortunes of the city’s neighborhoods, and with it the communities of Five Points and Whittier. Temple Emanuel, established in 1873, constructed its synagogue at Curtis and 24th Street in 1882, before removing to 16th and Pearl in 1899. Zion Baptist Church, originally established in 1865 and Colorado’s oldest African American congregation, first met for worship in a log structure located at the intersection of 20th and Arapahoe streets. The rapid development of Five Points and Whittier soon saw the establishment of the institutions that gave shape to each neighborhood’s character and community, including churches and synagogues, schools, hotels, restaurants, and drugstores, and other amenities and meeting places. In contrast to the mixed use for land in Five Points, Whittier was almost entirely residential from its inception, with its most significant commercial or business enterprises found in two-story brick corner stores and shops. Whittier became home to Denver’s second public park, Fuller Park, with a donation by its namesake, Horace Fuller, in 1879. Curtis Park became Denver’s first public park when a slightly less than two-and-a-half acre parcel, named for Samuel Curtis, was established in 1868. In time, the neighborhood would be home to such businesses as Kuner Pickle, Crescent Flour Mills, Colorado Iron Works, and the Denver Fire Clay Company, and these and other enterprises enjoyed ready access to the South Platte River and the Denver’s rail yard. In its earliest incarnation, Five Points was a mix of residential structures – some grand and stylish, others more modest – and business and industrial concerns. By August 1879, the Rocky Mountain News had started describing active building and development as Denver extended north and east along the South Platte River. ![]() As the historian William Allen West observes, the symbolism was unmistakable: with urban railroads and streetcars, Denver was not only of a sufficient size to warrant a transportation system, but also of a permanence to warrant the requisite investment. Transportation to these new neighborhoods was facilitated with the construction of urban railroads, and the city’s first street railroad, the Denver Horse Railroad Company, connected Five Points with downtown Denver in 1871. Similarly, Whittier’s major subdivisions were all platted from the late 1860s through the 1870s, a process begun in 1868 with Case’s Addition. ![]() In the late 1860s, Denver was enlarged with the addition of some 1600 acres and eight new subdivisions, three later within the bounds of Five Points (i.e., Case and Ebert’s Curtis and Clark’s and Shaffenburg’s ). ![]() Watchtower has been notifying you of potentially compromised logins for years, but we’ve always wanted to bring the full power of Watchtower’s desktop experience to mobile – so that’s just what we did. The size of the catheter (Fr) should be as small as possible to allow for smooth administration of the prescribed treatment, according to:.The scientific evidence that has studied PICC-related complications clearly shows that the number of lumens DOES matter and is a determining factor in the potential development of further complications. Although further studies are needed in this regard, preference is currently given to single-lumen CVCs, except if patient care and requirements necessitate a multi-lumen CVC 17. In addition, parenteral nutrition bags containing lipids should be infused through the larger lumen to reduce the risk of lumen obstruction 18-28. This is because if emulsions of nutrients, pharmacological agents or any parenteral infusion with a different pH come into contact, there is a higher risk of precipitates and, therefore, infectious complications. When a multi-lumen CVC is placed, it is recommended that a single lumen is used for parenteral nutrition (TPN). The review showed that findings from five randomised studies 27 documented that for every twenty single-lumen CVCs placed, a CRBSI would be avoided that would have occurred if multi-lumen CVCs had been placed. The meta-analysis showed that multi-lumen CVCs are not an independent variable for higher colonization or the occurrence of CRBSI. The problem was also assessed by means of a meta-analysis 26 and a review investigating both colonization rate and catheter-related bloodstream infection (CRBSI) in multi-lumen and single-lumen CVCs. Guidelines from the Centers for Disease Control and Prevention (CDC) 16 and a randomised clinical trial 25 indicated that when comparing multi-lumen central catheters (CVCs) with single-lumen central catheters, the former showed a higher incidence of infections. Thrombosis rates were statistically higher in the larger diameter catheters (p = 0.001). Grove and Pevec 24 reviewed the records of 678 patients with PICCs. Other researchers determined that a larger external diameter of the catheter increases the risk of venous thrombosis. ![]() Other statistically significant risk factors were noted, however the authors concluded that the number of lumens was not a factor in the development of thrombosis. ![]() Yi et al 23examined predictive risk factors for thrombus formation in patients with PICCs using ultrasound. ![]() Zochios et al 22 concluded that larger, multi-lumen PICCs demonstrated a higher incidence of thrombosis due to turbulence and restricted blood flow. The number of lumens and the size of the catheter have been described as risk factors for thrombus formation 21. Anticoagulation can be initiated, preventing further complications. Not all occlusions lead to upper extremity venous thrombosis (UEVT), and thrombotic occlusions can be treated with antithrombotic agents, while UEVT is treated symptomatically or with catheter removal. The literature describes occlusion rates in PICCs and links them to the number of lumens selected, reporting occlusion rates ranging from 7% to 34%, with fibrin sheath formation being a common cause of mechanical occlusion 20. Depending on the clinical situation, preference should be given to single-lumen devices as outlined in clinical practice guidelines 16-19. These findings have prompted a greater focus on ensuring appropriate use of the PICC in hospitalised patients, including recommendations on the “appropriate choice of vascular access type 16-17“.ĭouble and triple-lumen PICCs (Figure 2) are associated with an increase in occlusion. Ī recent study, for example, found considerable variation in the use of the PICC, indications for use and associated complications in a sample of 10 hospitals 15. However, the choice of device may in some cases be due to the limited information defining the appropriate indications for use of the single-lumen (Figure 1) versus the multi-lumen PICC 14. ![]() The first thing to remember is: I don’t sign in with my AppleID.
